1)First Download Form From Site
2)Fill The Form
3)Two Photo & Signature Required
3)Attach Address Proof & Dob N Name Proof With This Form
4)submit The Form To Nearest Ca Office And Required To Pay Rs 200 [Fees + Commission]
5)Receive Pan Card Within 15 Days By Speed Post

But point to be noted is that applying for PAN card is not a rocket Science and there is no need to get the services of a CA(+paying commission) when NSDL website is having all the details and the applicant would be spending lesser( I don't think more than Rs 130 including postage charges even after revision of charges)

So cumbersome even when applying via a CA? I feel better option is to get in touch with some roadside agent who would provide the applicant printed form + would fill the details himself + would collect the filled form as well as documents. All for Rs 200

Via Speed post? So it means you will be using UTI's service. My personal experience is that the overall service(Processing time, quality of the card/processing et al) of NSDL is way better compared to UTI, and they normally send PAN/PRAN cards via courier services.

Form printing charges not included in Rs 200

+ FYI, the IT Department has outsourced the task of printing cards to NSDL/UTI, and the cards are dispatched by those and not any (fully) government agency.
